DOSAGE AND ADMINISTRATION

Maximum frequency: Once per day.
Duration of Effectiveness: Up to 36 hours after dosing.

CONTRAINDICATIONS
Not for Use In:
Females or pediatric patients.
Patients on organic nitrates (risk of severe hypotension).

WARNINGS AND PRECAUTIONS
Hypotension: Avoid combining with alpha-blockers, antihypertensives, or substantial alcohol intake.
Priapism: Seek emergency care for erections lasting longer than 4 hours.
Vision Loss: Discontinue immediately if sudden vision loss occurs, which may indicate NAION (non-arteritic anterior ischemic optic neuropathy).
Hearing Loss: Sudden decrease or loss of hearing requires discontinuation and medical attention.
Pre-existing Conditions: Caution in patients with:
Cardiovascular disease.
Predisposition to priapism.
History of NAION or severe hepatic/renal impairment.
USE IN SPECIFIC POPULATIONS
Older Adults:
No major differences in safety or efficacy between individuals over 65 years and younger individuals.
Hepatic Impairment:
Use cautiously in mild to moderate impairment (Child-Pugh A/B).
Insufficient data for severe impairment (Child-Pugh C).
Renal Impairment:
Exposure increases in cases of reduced creatinine clearance (30–80 m L/min).
Higher exposure in end-stage renal disease on dialysis.
ADVERSE REACTIONS
Common Side Effects (>2%):
Headache, back pain, nasal congestion, flushing, myalgia, pain in limbs, dyspepsia.
Less Common Side Effects (<1%):
Gastroesophageal reflux, nausea, vomiting, upper abdominal pain, muscle spasms, and arthralgia.
Back Pain/Myalgia:
Occurs 12–24 hours post-dose, resolves within hours or days.
Usually mild to moderate; severe cases are rare (<5%).
Management:
Use of acetaminophen or NSAIDs is generally effective.
Narcotics are rarely required.
DRUG INTERACTIONS
Potential Interactions with Tadalafil:
Nitrates:
Contraindicated; severe hypotension may occur. Wait at least 48 hours before administering nitrates post-Tadalafil.
Alpha-Blockers:
Increased risk of blood pressure drops.
Alcohol:
Large quantities combined with Tadalafil may cause dizziness, headache, and orthostatic hypotension.
Antacids:
May slow Tadalafil absorption but does not reduce effectiveness.
CYP3A4 Inhibitors:
Drugs like ketoconazole, erythromycin, and grapefruit juice increase Tadalafil exposure.
CYP3A4 Inducers:
Rifampin reduces Tadalafil exposure by 88%.
OVERDOSAGE
Symptoms: Similar to lower doses but may require medical attention.
Treatment: Supportive measures; hemodialysis is ineffective in eliminating Tadalafil.
